The OptiPillows EPAP mask has been upgraded. Now you have a choice: the Original OptiPillows EPAP mask, or the deluxe upgraded version with a simple air blower added to make it function like a CPAP machine.
The new system functions like a continuous positive airway pressure (CPAP) machine: the EPAP mask provides pressure during expiration and the air blower provides pressure during inspiration. This is the closest anyone can get to keeping breathing very close to normal — no more waking up to a CPAP machine blowing very hard and disturbing your sleep.
The use of the air blower is optional. It may be most helpful for new users of the EPAP mask, in particular during the first week or two while getting used to the mask. Thereafter, you can detach the air blower and use the EPAP mask without it — it is your preference. Use it as a CPAP system or as an EPAP device.

How EPAP works

The pressure during expirations is set with the proprietary valve of the OptiPillows EPAP mask, while the pressure during inspiration is set with the fan speed of the air blower.

Sal, the owner, is very friendly and you can tell he truly cares about his customers. 5 star service to say the least! I was traveling out of town and unfortunately after my first night the bracket to my CPAP hose broke. My machine would then not sufficiently blow the proper amount of air to treat me. I panicked for a bit, but then I looked online and found this place 15 minutes from where I was staying. The owner was so understanding and accommodating and had the replacement piece in stock! Problem solved anxiety gone! His prices were very good as well. Everything you need to know about EPAP

A growing body of evidence supports the idea that high inspiratory pressure is not always necessary.

  1. More than a dozen studies have shown that EPAP devices are effective for many — but not all — patients (Berry et al., 2011; Kryger et al., 2011), indicating that inspiratory pressure is not essential.
  2. It was proposed that CPAP devices should allow users to set inspiratory pressure below expiratory pressure (Hakim et al., 2015). Traditionally, inspiratory pressure is set equal to or higher than expiratory pressure.
  3. Recent expert reviews concluded that CPAP therapy has been managed incorrectly for years (Brusie C., Sleep Review, 2022).
  4. For patients with obstructive sleep apnea, adequate treatment can be achieved when inspiratory pressure is set slightly lower than expiratory pressure. This approach, termed Inspiratory Pressure Relief (IPR), contrasts with the well-known Expiratory Pressure Relief (EPR) (Farney et al., 2024).
  5. Additionally, one study demonstrated that setting inspiratory pressure significantly lower than expiratory pressure performs as effectively as conventional CPAP (White et al., 2024).
